The reality is that we are more than just a physical body. In order to help you achieve the most positive change in your health in the shortest amount of time, its important for me to get the whole picture of what your health status is today. This is how we will measure your progress.
Please print and completely fill out (to the best of your ability) all the initial evaluation documents, 1-5 below. Bring the completed forms with you to your session.
